Job Posting: Speech Language Pathologist (SLP)

Job Summary:
We are seeking a compassionate Speech Language Pathologist (SLP) to provide high-quality speech therapy to patients in the comfort of their homes. The SLP will work under the supervision of the Director of Clinical Services/Clinical Manager to administer physician-ordered therapy services, aiming to improve communication and help patients return to an optimal level of functioning. As a critical member of the home healthcare team, the SLP will also collaborate with other professionals in patient care planning and progress evaluation.

Responsibilities:

  1. Provide speech therapy services as prescribed by a physician, with the goal of improving or maximizing the patient’s communication skills.
  2. Assist patients in returning to their optimum and productive level of living within their capabilities.
  3. Participate periodically in patient care planning and collaborate with the interdisciplinary care team.
  4. Perform all skilled speech therapy procedures in accordance with the treatment plan.
  5. Supervise and direct personnel, including home health aides, as assigned.
  6. Conduct initial evaluations, take patient history, and develop treatment plans.
  7. Consult with physicians to modify treatment plans as necessary.
  8. Write detailed reports for physicians on patient progress and treatment outcomes.
  9. Instruct patients and their families on home programs and exercises to continue progress outside of therapy sessions.
  10. Provide in-service training to Agency staff on speech therapy techniques.
  11. Complete initial evaluations, submit treatment plans and goals to physicians for approval, and conduct periodic recertification every 60 days.
  12. Write progress notes for each patient visit, and complete a reevaluation when the expected duration of treatment is reached.
  13. Prepare a comprehensive summary upon patient discharge.
  14. Conduct supervisory visits to oversee care provided by home health aides, ensuring quality and compliance.
  15. Assist physicians in evaluating the patient’s level of function and recovery progress.
  16. Contribute to the development and revision of care plans as needed.
  17. Maintain accurate clinical documentation, including progress notes, and ensure timely submission of all required reports.
  18. Advise and consult with families and other Agency personnel on communication issues and therapy progress.
  19. Participate in Agency inservice programs and provide input on training needs.
  20. Complete and submit OASIS assessments, transfers, resumptions of care, discharges, and reports of significant changes in the patient’s condition in accordance with Agency timelines.
  21. Appropriately utilize ICD-10 codes for accurate diagnosis and billing.
  22. Engage in Quality Assessment and Performance Improvement (QAPI) activities as assigned.

Working Environment:

  • Primarily works in Agency offices and patient homes.
  • Travel to and from patient homes is required.

Job Relationships:

  • Supervised by: Director of Clinical Services/Clinical Manager
  • Supervises: Home Health Aide(s) as assigned

  • High risk

Lifting Requirements:

  • Ability to engage in physical activity and perform moderate lifting, bending, and standing for extended periods.
  • Must be able to lift and assist patients as required during therapy sessions.
Requirements:

Qualifications:

  • Master’s or Doctoral degree in Speech Language Pathology.
  • Must be licensed as a Speech Language Pathologist in the state(s) where services are provided.
  • Alternatively, completion of 350 clock hours of supervised clinical practicum and at least nine months of full-time SLP experience, followed by successful completion of a national examination approved by the Secretary.
  • Two (2) years of experience preferred.
  • Strong ability to work independently and as part of a team.
  • Ability to communicate effectively with patients, families, and other healthcare professionals.
